Insurance Accepted

At the time of your visit, please provide our office with the correct insurance information, including your insurance card to be copied. We ask that all co-pays be paid at the time of service. We will perform courtesy billing for you, provided you have given us all pertinent billing information.

Your insurance coverage is an agreement between you and your insurance company. Financial responsibility rests with the patient for deductibles, co-insurance and non-covered services. We accept Visa, Mastercard and Discover.

Regarding Pacific Source Community Solutions Insurance:

Dear Patient,

We are writing to inform you that Salem Gastro and Salem Endoscopy Center’s insurance network has changed. As of January 1, 2026, we will no longer be in-network with Pacific Source Community Solutions. We will no longer accept this insurance.

We made this difficult decision after several months of attempting to reach an agreement with Pacific Source Community Solutions.

We are providing advance notice so you can make the best decision for your health care moving forward. Your health is our priority, and we are here to support you in this transition.  If you choose to have your medical care transferred elsewhere, you may contact our Medical Records department and sign a release.

Though we cannot advise you what insurance carrier is best for you, please contact our office if you have any questions regarding our network status with any other insurance company if you choose to change your carrier.
